You agree to use this website only for lawful purposes and in a manner that does not violate any applicable law, regulation, or the rights of Revive or any third party.
2. Approved Purchasers Only
This website is not intended for public retail sales. Products offered through this website are available only to approved licensed healthcare providers and medical practices that have been approved by Revive in its sole discretion.
By accessing or purchasing through this website, you represent and warrant that you are an approved licensed healthcare provider or medical practice, or are acting on behalf of an approved licensed healthcare provider or medical practice, and that you are authorized to place orders on behalf of the applicable practice or provider.
Revive reserves the right to require credentialing information, licensure information, practice information, resale documentation, or any other information it deems necessary to verify purchaser eligibility. Revive may suspend, restrict, or terminate access to the website or reject any order if purchaser eligibility cannot be verified or maintained.
3. Product Information
Revive strives to ensure that product descriptions, images, specifications, pricing, and availability are presented as accurately as possible. However, Revive does not warrant that all product information, content, or other materials on the website are complete, current, accurate, or error-free.
Product appearance, packaging, labeling, specifications, and availability may change at any time without notice, including changes made by the applicable manufacturer.
Revive reserves the right to correct any errors, inaccuracies, or omissions and to update information or cancel orders if any information on the site is inaccurate at any time, including after an order has been submitted.
4. Orders and Acceptance
All orders placed through the website are subject to review and acceptance by Revive. Submission of an order does not guarantee acceptance or fulfillment. Revive reserves the right, in its sole discretion, to refuse, limit, cancel, or delay any order for any reason, including product availability, pricing errors, suspected fraud, compliance concerns, credentialing issues, shipping limitations, or payment issues.
If an order is canceled after payment has been processed, Revive will issue an appropriate refund in accordance with its internal procedures.
5. Pricing and Payment
All prices are subject to change without notice. Revive reserves the right to correct pricing errors at any time, including after an order has been placed.
Payment is due and collected at the time the order is placed unless otherwise agreed in writing by Revive. By submitting payment information, you represent that you are authorized to use the selected payment method and authorize Revive to charge the applicable amount for your order, including applicable taxes, shipping charges, and any other disclosed fees.
6. Sales Tax
Applicable sales tax will be charged based on the ship-to location, purchaser location, or other legally relevant point of sale, as determined by Revive in accordance with applicable law. Purchaser is responsible for all sales, use, excise, and other applicable taxes, duties, or governmental charges associated with the purchase of products, except for taxes imposed on Revive’s net income.
If purchaser claims exemption from any sales or similar tax, purchaser must provide a valid tax exemption certificate or other documentation acceptable to Revive before the order is processed. Revive reserves the right to reject any exemption documentation that is incomplete, invalid, or not accepted under applicable law.
7. Shipping Terms
Revive offers free ground shipping on all domestic orders. Customers requesting expedited shipping will be charged the cost of expedited shipping.
Revive assumes no liability for additional costs, losses, or damages resulting from delayed deliveries. All shipments are FOB Freight Prepaid at the manufacturer’s facility, at which point title to the products and all risk of loss pass to the purchaser.
Delivery dates and shipping timeframes are estimates only and are not guaranteed unless expressly confirmed by Revive in writing.
8. Returns and Product Replacement
Payment is due at the time the order is placed.
All standard sales orders are final, and products are non-returnable.
Revive, through the applicable manufacturer, will replace defective product when Purchaser provides timely notice of the defect, provided the product has not been opened, expired, or damaged.
Purchaser is responsible for promptly inspecting all products upon receipt and notifying Revive of any alleged defect, shortage, shipping error, or discrepancy within the timeframe required by Revive or the applicable manufacturer. Revive will coordinate any approved replacement with the manufacturer. Replacement eligibility remains subject to Revive review and applicable manufacturer requirements.
Except as expressly stated above, no returns, credits, refunds, or replacements will be provided for standard sales orders.
9. Inspection of Orders; Damaged or Incorrect Shipments
Purchaser is responsible for inspecting all products promptly upon delivery. Any claim for shortage, shipping error, visible damage, defect, or other discrepancy must be reported to Revive promptly upon receipt.
Failure to timely inspect and report such issues may result in denial of the claim, replacement request, or other requested resolution.
10. Product Complaints, Adverse Events, and Reportable Issues
Purchaser must promptly report to Revive any product complaint, suspected defect, adverse event, malfunction, product quality issue, labeling issue, or other reportable event relating to any product purchased through the website.
For purposes of these Terms, a product complaint may include, without limitation, any concern regarding product identity, quality, performance, packaging, labeling, sterility, damage, breakage, malfunction, or suspected failure to perform as intended. An adverse event may include any patient injury, unexpected outcome, or other event that may require reporting to the manufacturer, regulatory authorities, or other parties.
Purchaser agrees to cooperate fully with Revive and, if applicable, the manufacturer in connection with the review, investigation, documentation, retrieval, return, or reporting of any such issue. Purchaser shall maintain and provide relevant information upon request, including product name, lot number, serial number if applicable, date of receipt, date of use, description of the event or complaint, photographs if available, and any other information reasonably requested.
Products that are the subject of a complaint, suspected defect, or adverse event should not be returned, discarded, altered, relabeled, or destroyed unless and until Revive or the applicable manufacturer provides instructions.
Submission of a product complaint does not guarantee return eligibility, replacement, or credit, all of which remain subject to Revive review and applicable manufacturer policy.
11. No Medical Advice; Purchaser Responsibility
The content on this website is provided for general informational purposes only and does not constitute medical, clinical, reimbursement, legal, or regulatory advice. Revive does not provide patient-specific recommendations or medical direction through this website.
Purchaser is solely responsible for evaluating the suitability, use, handling, storage, and application of any product purchased through the website and for ensuring compliance with all applicable laws, regulations, licensing requirements, manufacturer instructions, and professional standards.
12. Disclaimer of Warranties

Revive’s total liability for any claim arising out of or relating to any product purchase or use of the website shall not exceed the amount actually paid by the purchaser to Revive for the specific product giving rise to the claim.

Free ground shipping on all domestic orders. AlloClae orders are subject to standard overnight shipping. Expedited shipping beyond what's provided is charged at cost. Revive assumes no liability for additional costs or damages from late deliveries. All shipments are FOB Freight Prepaid at the manufacturer's facility, at which point title and risk of loss pass to the purchaser.
